Accepted forms of payment:
Certified check, money order, or personal check (which must
be accompanied by proper valid ID). No second party checks are
accepted.
- Local Procedure
• Accepts in PERSON only: I-90 to
replace a lost, damaged, or incorrect I-551.
• Accepts by MAIL or PERSON (if in person, need an INFOPASS
appointment):
- I-821 with I-765
- N-300
- N-470
- N-565
- N-600
• All adoption forms (I-600 and I-600A) must be MAILED
to the Boston District Office (as of 3/1/04).
• All I-485 and applications filed in conjunction (I-131
and I-765) must be MAILED to:

• I-131 (Travel Authorization): For emergency travel,
you must apply in PERSON and bring documentary evidence of the
emergency. Processing times vary.
- Additional Information
• Change of address for naturalization
(N 400) may be completed over the phone (1-800-375-5283), everyone
else must use the form and send it to the office where their
application is pending.
• Fingerprints notices are sent after you have applied.
You will be notified as to which Application Support Center
to go to and when.

• If I-693 is required (medical examination)
you MUST include it at the time of filing I-485.
• I-765 (first time and renewals) are mailed to the Vermont
Service Center; unless filed w/ I-485 at this office; then it
will be processed at this office concurrently w/ I-485.
• Advance parole applications need to be mailed to the
Portland District Office. This office needs at least 10-14 days
ahead of intended travel.
• For emergency travel call the National Customer Service
Center 1 800 375 5283.

- 2 new passport style photos and a valid passport
- If you do not have a valid passport, bring 3 passport-style
